Computer models, as roughbarked said. Based upon recorded data from previous weather patterns, and probably updated and revised regularly with new observations and data.

To be more specific, solution of the Navier-Stokes equations for fluid flow in a rotating reference frame coupled with equations for heat transfer, turbulence, and condensation.
